Our Year 6 leavers move on from Meare Village Primary School to a range of schools in the local area and beyond.

Our transition work begins early in the Summer Term, with key identification of what support each child will need to make their transition as smooth as possible.

This will involve:

• meetings about SEND needs
• extra visits to the next school (often with a familiar member of staff from our setting)
• key staff from the next school visiting us here
• booklets with photos and key infomation given to the child
• supportive meetings with families

Leaving primary school is both challenging and exciting – we try to make it the best experience for each and every child!
